
About shoulder pain from different reasons
scalene muscle syndrome.
The patient has had pain in both shoulders for over 3 months. Her GP suggested a blood test to rule out rheumatoid arthritis, and the result came back negative.
She experiences severe shoulder pain, especially at night, and sometimes has shooting pain that radiates from the left axillary area down to the hand.
During our examination, we found that her scalene muscles on both sides were tight and tender. When pressure was applied to the scalenes, the sharp pain radiated to her shoulders and arms. Based on the findings — tenderness, radiating pain, and night pain — we concluded that her shoulder pain is due to scalene muscle syndrome.
After acupuncture at the Tianchuang (天窗) point, her shoulder pain was immediately relieved.

quadrangular space syndrome
He has had shoulder pain for over a year. The pain is constant, with sharp, stabbing sensations at night. This kind of sharp pain at night suggests nerve involvement. So, we performed an examination to identify which nerve might be affected.
I checked the brachial plexus, but found no signs there. However, when I examined the axillary nerve, he reported increased pain at the front of the shoulder and more pain throughout the shoulder area. The whole shoulder was sensitive. This suggests that the axillary nerve injury is likely the main cause of his shoulder pain.
The axillary nerve controls the teres minor and deltoid muscles. Injury to the nerve can make these muscles tight and painful. So, I believe his shoulder pain has two key causes: nerve injury and muscle involvement. They influence each other. Therefore, to relieve his pain, we need to treat both the nerve and the muscles.
The axillary nerve passes through the quadrangular space, which corresponds to the Jianzheng (肩貞) point in Chinese medicine. Treating this area can help address both the teres minor injury and the axillary nerve damage.
He hasn’t felt improvement from other treatments like physiotherapy or acupuncture, because those treatments did not identify the real issue — the combination of nerve and muscle dysfunction.

The patient has experienced right shoulder pain for the past two years. He initially came to see me for sleep issues, which significantly improved after several acupuncture sessions. As a result, he asked if I could also help with his longstanding shoulder pain.
Upon examination, there was no local tenderness in the shoulder. However, he reported increased pain during shoulder extension. Interestingly, when I applied pressure to the neck area near the Tianchuan point (SCM-scalene region), the pain radiated directly to the right shoulder.
I performed acupressure on the Tianchuan point, and the shoulder pain disappeared immediately.
This is a typical presentation of scalene muscle syndrome, where shoulder pain is actually referred nerve pain caused by tension or compression from the scalene muscles.
