Sharp Medial Knee Pain? The Problem May Not Be Inside the Knee

This patient had been suffering from severe right medial knee pain for about two months.
The pain was especially sharp when moving from sitting to standing.
Interestingly, once he started walking, the knee often felt slightly better.
At night, however, the pain became much worse when lying on the affected side.
He had also started walking with a limp.
At first glance, this looked like a typical knee joint problem.
The patient had already undergone X-rays and used medication, but improvement remained limited.
The interesting part appeared during examination.
There was marked tenderness around the right Baichongwo region.
When pressure was applied there, the pain radiated directly into the medial aspect of the knee.
👉 This pain pattern did not behave like a simple joint problem.
Instead, it strongly suggested irritation along the saphenous nerve pathway, especially involving the infrapatellar branch.
The treatment therefore focused on the nerve irritation source rather than the knee joint itself.
Treatment included:
- targeted acupuncture with a single needle
- local bloodletting
- cupping release around the Baichongwo region
The response was significant.
After treatment, the pain reduced markedly.
Following four treatment sessions, the patient later shared this feedback:
“After 4 sessions my knee is 99% better.

Clinical Insight
This case highlights an important clinical reality:
Not all medial knee pain originates from structures inside the knee joint.
When pain is:
- worse after sitting
- triggered when standing up
- aggravated by side lying
- partially relieved by walking
the problem may involve nerve irritation rather than joint degeneration.
Many chronic knee pain cases continue being treated as “joint problems” even when imaging findings remain minimal.
But clinically, the pain generator may actually lie along the nerve pathway itself.
The key is not simply identifying where the pain is felt.
The key is identifying which structure continues irritating the nervous system.
