中医治愈病例分析

这里再介绍我近期用针灸、中药等成功治愈的二个病例,一来让大家分享我成功治愈病人后的喜悦,二来让大家了解一些医学、针灸和中医方面的知识,了解什么病症可以针灸中医治疗,了解诊断和治疗疾病是一个很复杂、很高深的学问,不是门外汉想的那么简单。
患者Ema 女,20岁,中国留学生。因双手麻木二个月来诊。麻木夜间加重,因麻木而影响睡眠。患者因怀疑自己患了颈椎病而恐慌,家长也劝她快点回国,以免延误治疗。患者因看到我的文章,并有朋友推荐,知道我在神经、骨伤方面有经验,而来我处就医。我详细询问了患者的发病过程,并给予仔细的神经系统查体。上肢的腱反射、感觉未发现异常,颈椎检查也未发现神经根刺激征。从以上检查我基本排除了颈椎病的诊断。再检查患者的后背,发现颈、肩、后背的肌肉非常紧张,并能触摸到皮下条索状硬结,患者有肌肉劳损的情况。根据患者的发病过程、临床表现及我的检查,判断患者的双手麻木是有引起肩背筋膜炎的。
按肩背筋膜炎针灸治疗一个疗程,并给予活血通络的中药,患者的双手麻木消失了。她高兴地告诉她国内的父母,双手麻木只是后背、颈部的肌肉劳损和炎症引起,而不是颈椎病等压迫神经,她可以安心地继续读书了。
患者的肩背筋膜炎是有长期做计算机工作引起的。长期一个姿势的工作,致使肩背肌肉紧张,紧张的肌肉使局部血液循环减少,长期的血液循环减少使肌肉纤维发生炎症,炎症致使肌肉纤维的增生,长期的纤维增生产生条索样硬结,条索样硬结进一步刺激肌肉紧张,形成局部的恶性循环。早期的预防和积极治疗可阻断恶性循环的发生。后背肌肉的紧张和条索状硬结可刺激脊柱旁的交感神经链,因此患者感觉双手麻木,上肢发冷、发胀,但医生给予神经系统查体时并未发现异常,这可以与颈椎病相区别,这也是交感神经链损伤的一个特点。因交感神经受刺激患者可有咽干、胸闷、头晕、头痛、乏力等症状。虽然肩背筋膜炎不是很严重的病症,但患者非常痛苦,并且易与其它疾病混淆,易造成误诊、误治。针灸治疗可使患者的后背肌肉放松、并可消除无菌性炎症,中药可活血通络,达到治疗此病的目的。但要想完全彻底地治愈此病,还要需要患者的配合。长期一个姿势工作的人,须不间断的变换姿势;可以工作30—40分钟,然后站起来活动活动上肢;有规律地参加一些体育活动,如游泳、打球、跑步,实在没有时间也可以在家跳跳绳、做做俯卧撑、仰卧起坐等活动,这些都会对此病有帮助。如果患者的病情太重,治疗效果不佳,经过预防、锻炼仍不见好转,也许应改换一个适合的工作。
患者路先生,80岁,因走路迈不开步20多天而来就诊。患者从前有腰腿痛病,这次患者走路困难,患者和家属都认为是腰腿痛病加重引起,因此随去推拿治疗,腰腿痛有所减轻,但仍走路迈不开步伐。经朋友介绍来到我的诊所。检查时,让患者走直线不能,让患者单腿站立不能。神经系统查体时还发现,左鼻唇沟稍变浅,伸舌稍左偏,左腹壁反射稍减低,左膝反射稍活跃,左巴彬氏征阴性,因此我怀疑大脑有病变。再询问患者的病史,知道二年前有一次轻微的“中风”,当时只有语言不清,二个月后恢复正常。因此,根据病史和神经系统检查我诊断为脑中风(腔隙性脑梗塞)。这个病例是脑中风影响了患者大脑的锥体系统和锥体外系统,造成患者走路步伐小,走直线不稳的,其实与患者的腰腿疼没有一点关系。
给予针灸和中药治疗。针灸以头针为主,中药以补阳还五汤加减,三个星期后,患者可走直线,可单腿站立,走路可迈开步伐,基本恢复正常。患者初诊时给我的第一感觉也是想到是不是腰腿病,但我神经系统检查后排除了此想法。仔细的神经系统查体让我发现了有意义的体征,让我想到脑中风的可能。如果我没有神经内科的基础,也许我发现不了这些体征,因为患者的症状太轻微了。如果患者有偏瘫、失语等典型症状,诊断就一目了然,也不需要我再费口舌了。如果不按脑中风用头针和中药治疗,而按腰腿疼治疗只是局部的针灸、推拿,我想患者是不会有很好的治疗效果的。这也是我一再强调的明确诊断的重要性。
无论是病例一的手麻,还是病例二的走路不稳,都是疾病的一个症状,都是患者亲身感受到的痛苦,究竟是什么疾病患者不很明了,但是患者会根据症状联想到最常见、最普通的疾病,或者最严重的疾病。手麻会想到颈椎病,走路不稳就想到已经患的腰腿疼,这是正常的思维,但医生一定要打破这种习惯思维,利用医学知识,正确判断患者的病情,选择合适的治疗。(浏览更多医学文章,请登陆 www.drwin.co.nz 黄博士诊所095296185)

My back, neck, upper limbs and lower limbs have been in pain (aching) for a very long time. My background is nursing and business. Much of my nursing life required me to do heavy lifting which resulted in back, neck and shoulder pain. I have spent a fortune on massage, chiropractic care and pain relief all of which has helped. Fourteen months ago I suffered from a major stress episode when my partner went missing at sea. He did come home thank goodness after being shipwrecked. The week after my partner came home my neck stiffened and I could not turn it very far left or right and my back tension was very strong and I have had very little relief. Funny how the stress comes out after the event. I went to see Dr Win Huang at the end of January 2010 in the hope he could help me. I have had between 2 and 3 treatments a week and the muscles in my back are now relaxed. I am able to turn my head with increased range. The pain radiating down both my arms and wrists is much reduced. My partner tells me I stand straighter. I am aware I need a few more acupuncture sessions and that acupuncture is not an ongoing forever requirement. The immense reduction in pain and improvement in flexibility already gained has me want to share the care Dr Win Huang can provide. I am so privileged and grateful that I have met Dr Win Huang.

5 Steps to a Healthy Heart with Acupuncture

Newsletter JulyFebruary is the American Heart Association’s Heart Health Awareness Month, emphasizing the dangers of heart disease and the importance of heart health.

Heart disease includes conditions affecting the heart, such as coronary heart disease, heart attacks, congestive heart failure, and congenital heart disease. Despite dramatic medical advances over the past fifty years, heart disease remains a leading cause of death globally and the number one cause of death in the United States.

1. Manage High Blood Pressure
High blood pressure makes the heart work harder, increasing its oxygen demands and contributing to angina. This excessive pressure can lead to an enlarged heart (cardiomegaly), as well as damage to blood vessels in the kidneys and brain. It increases the risk of heart attacks, stroke and kidney disease.

Acupuncture has been found to be particularly helpful in lowering blood pressure. By applying acupuncture needles at specific sites along the wrist, inside the forearm or in the leg, researchers at the Susan Samueli Center for Integrative Medicine at the University of California, Irvine, were able to stimulate the release of opioids, which decreases the heart’s activity and thus its need for oxygen. This, in turn, lowers blood pressure.

2. Quit Smoking
Most people associate cigarette smoking with breathing problems and lung cancer. But did you know that smoking is also a major cause of coronary artery disease? In fact, about twenty percent of all deaths from heart disease are directly related to cigarette smoking.

Acupuncture has shown to be an effective treatment for smoking. Acupuncture treatments for smoking cessation focus on jitters, cravings, irritability, and restlessness; symptoms that people commonly complain about when they quit. It also aids in relaxation and detoxification.

3. Maintain a Healthy Weight
Obesity is associated with diabetes, high blood pressure and coronary artery disease, all of which increase the risk of developing heart disease, but studies have shown that excess body weight itself (and not just the associated medical conditions) can also lead to heart failure. Even if you are entirely healthy otherwise, being overweight still places you at a greater risk of developing heart failure.

Acupuncture and Oriental Medicine are an excellent adjunctive tool when it comes to losing weight. They can help to energize the body, maximize the absorption of nutrients, regulate elimination, control overeating, suppress the appetite, and reduce anxiety.

4. Reduce Stress
Stress is a normal part of life. But if left unmanaged, stress can lead to emotional, psychological, and even physical problems, including heart disease, high blood pressure, chest pains, or irregular heart beats. Medical researchers aren’t sure exactly how stress increases the risk of heart disease. Stress itself might be a risk factor, or it could be that high levels of stress make other risk factors worse. For example, if you are under stress, your blood pressure goes up, you may overeat, you may exercise less, and you may be more likely to smoke.

Numerous studies have demonstrated the substantial benefits of acupuncture in the treatment of stress, anxiety and mental health. In addition to acupuncture, Oriental medicine offers a whole gamut of tools and techniques that can be integrated into your life to keep stress in check. These tools include Tui Na, Qi Gong exercises, herbal medicine, dietary therapy, meditations and acupressure that you can administer at home.

5. Improve Sleep
Poor sleep has been linked with high blood pressure, atherosclerosis, heart failure, heart attacks, stroke, diabetes, and obesity. Researchers have shown that getting at least eight hours of sleep is needed for good heart health and getting less than eight hours of sleep can put you at a greater risk for developing heart disease.

Acupuncture has shown great success treating a wide array of sleep problems without any of the side effects of prescription or over-the-counter sleep aids. The acupuncture treatments for problems sleeping focus on the root disharmony within the body that is causing the insomnia. Therefore, those who use acupuncture for insomnia achieve not only better sleep, but also an overall improvement of physical and mental health.

Tips for Finding an Acupuncturist

When seeking out a professional acupuncturist, it is important to ensure that the acupuncturist is licensed and certified. Sterilising procedures should be used and more often than not, your acupuncturist will likely use disposable needles. If you are uncomfortable with the idea of needles, an acupuncturist may provide alternatives such as herbal methods, magnetic needles and ear cups. These are thought to provide similar results although are not considered as effective as acupuncture that utilises needles.
Whatever path you take to treating your problem, you can be relieved to know that you have many options, particularly alternative ones such as acupuncture. Hopefully, you will find a qualified acupuncturist who can listen to your concerns about the problem and begin a treatment plan that provides effective relief.

肠易激综合证中医治疗一例

近期治疗一例“肠易激综合证”病例,效果非常明显,我让患者把他的的病情和治疗感受写了下来,与大家一起分享他的喜悦和对中医的信任。

“Began approx.1 year ago (Jan/2009)during hottest part of summertime. I was working hard at the time and under stress, initial symptoms were lack of appetite and not liking food.

Despite finishing that work in Feb, symptoms continued to worsen thorough out the year 2009.appetite was not good and I started having problems urinating. Especially in the wintertime when it was cold, I had sudden desire to go to the toilet –like an emergence. if I could not go to the toilet immediately it would hurt very much in my penis.

Other symptoms that have developed over the past year:

* Not liking the tast of most foods (even things I used to love eating)
* Most food feels very “dry” in my mouth and difficult to chew and swallow.
* I can only eat very small meals at one time.
* about five minutes after eating ,I feel sore/sick in stomach. about 20 minutes after eating, I suddenly feel very ”empty” and “hollow” inside my stomach.
* Often I feel start to feel dizzy about 5-10 minutes after eating.
* When I feel hungry, it is very painful and sudeen.
* Wake up in the middle of the night covered in sweat and have to urinate urgently.
* Feeling tired and low energy.
* Sometimes stools are very loose, sometimes Ok. Because I eat 5–6 small meal every day, I need to go to the toilet up to 5 times a day to pass stools.

Morning times are usually the worst. My appetite tends to improve in the afternoon and evening.

Jan 27(2010):Feeling very tired, sore in stomach, passing many small stools(approx.6—7 times/day),no appetite but get sudden feeling of intense hunger.
(first day of treatment, receive little acupuncture, begin taking herbs)
feeling good immediately after treatment, appetite has returned but after arriving home and eating I feel very sore in stomach again.

Jan 28: Morning : feeling very sore in stomach, dizzy, lightheaded.
Afternoon: improving. Evening: feeling Ok. sleep well. No night sweats.

Jan 29: wake up feeling very good. This continues all day. Appetite good, stools good, stomach feels normal, digestion is good . I have more energy.

Jan 30: still a little sore in lower bowels in morning before passing stool but OK apart from that, Generally feeling very good.

Feb 1: Finish herbs this morning. Feeling very high energy all day (almost manic).

Feb 2: feel slight symptoms in morning (empty, hollow feeling in stomach after eating).Get more herbs and acupuncture

Feb 3: feeling good in stomach, stools slight soft and runny, feeling slighty tired, sleeping well.

Feb 4: slighty liquid stools feel unsettled in stomach. Have a acupuncture in Dr Win Clinic.

根据患者的症状和检查我诊断为“肠易激综合证”。肠易激综合征(IBS)是胃肠道功能紊乱的一种表现,是胃肠道最常见和最典型的功能性疾病,临床表现为,腹疼、腹胀、排便习惯和大便性状异常持续存在或间歇发作的一组综合征。本病的病因尚不明确,精神因素和饮食因素可能是两大促发因素。

从我们的经验看,有这种病的人,多数有精神因素、食物生冷、食物过敏的历史。与中医的肝郁脾虚征有相同的病机,而抑肝扶脾法可以明显地改善此症状。我选择中药和针灸治疗这种疾病 ,中药用炒白术15克、炒白芍15克、佛手10克、陈皮10克、防风6克、郁金10克、甘草6克、柴胡10克、煨木香10克(后下)、山楂10克、煨葛根10克、独活15克、龙骨10克、牡蛎10克水煎服。连用7—14剂。针灸以胃经穴位和头部穴位为主。(请浏览 www.drwin.co.nz 黄博士诊所 09 5296185)

ACC在针灸方面的政策改变

ACC的政策调整是近期的热门话题,我也接到许多朋友的电话,询问ACC的政策改变对针灸的影响,为了大家更好的使用ACC,也表达我对朋友关心针灸的谢意,有必要再谈谈ACC在针灸方面的政策改变。
新年开始,每个针灸医生都受到ACC的一封信,告诉大家ACC在针灸政策上的暂时改变,大体有以下二个方面。第一,针灸治疗一个疗程后(如肩部扭伤,ACC规定针灸治疗12次),如果患者受伤的病还没有治疗痊愈,ACC只允许最多再申请6次针灸治疗。以前,一个疗程的针灸治疗后,如果受伤的病还没有痊愈,需要继续针灸治疗,针灸医生需填写一个ACC32表,再为患者申请一定的针灸治疗,究竟多少治疗次数,根据患者的病情和医生的判断来决定,也可能5次,也可能12次。但是,现在最多再申请6次针灸治疗,并且ACC的审查更加严格。第二,以前,针灸医生为患者申请增加针灸治疗,填写ACC32表,ACC是付一定费用给针灸医生,而现在,一切都是义务的,没有费用。
从以上ACC政策的改变,我们可以看出,对ACC患者影响还是很大的。这就要求患者,在得到伤害后,选择合适的针灸治疗,在最短的时间,以最小的治疗次数,把病痛治疗痊愈。不要存在任何的侥幸心理,第一个疗程治疗不好,还可以申请第二个疗程。对针灸医生填写ACC32表,要求更严格,填写更仔细,并且是义务填写。说明ACC要从一切方面节省资金,来填补ACC以前很大的资金缺口。
很多朋友把针灸与理疗弄混淆,向我打听治疗费减半的问题。针灸是中医治疗,英文是ACUPUNCTURE ,指针灸师用银针在穴位上针刺,只要在ACC注册的针灸师都可以治疗ACC的病人;理疗是一种西医范畴的治疗方法,英文是PHYSIOTHERAPY,一般是指理疗师用理疗仪器(如中频理疗仪、红外线和康复仪器等)或理疗医师的手法治疗患者。从01/11/2009,ACC不再付给理疗师全额治疗费,只付一半治疗费用,因此患者要交付一定的治疗费。ACC的这些改变只针对理疗,而不适用于针灸治疗,那些对针灸和理疗有混淆的朋友们,如果有意外受伤,可以放心去针灸治疗。有些针灸医师对ACC患者也有额外收费,这与ACC的政策改变无关,而是针灸医师的自行决定,有的诊所收费是10纽币,有的收费是15纽币,这与诊所的收费标准不同而不同。
意外的身体和精神的损害都属于ACC报销范围。它包括1)骨折与错位,如颅骨骨折、腰椎骨折、四肢骨折、肩关节脱位等;2)烧伤,如眼烧伤、身体各部位烧伤等;3)与工作相关逐渐起病,如腕管综合症、肩周炎、网球肘、肌腱炎、腱鞘炎、反射性交感神经营养不良等;3)扭伤,如急性腰扭伤、坐骨神经痛、韧带拉伤、半月板损伤、肩扭伤、颈扭伤及身体各部位的扭伤;4)撕裂伤,如内脏的裂伤及身体各部位的撕裂伤等;5)挫伤及挤压伤;6)创伤性截肢;7)其他,如脑震荡、脑震荡后遗症、虫叮咬伤、过敏反应、椎间盘突出、疝气、性虐待、强奸等。患者有以上意外伤害都可以针灸治疗。
怎样得到ACC针灸治疗。按照我下面的操作,而得到ACC针灸治疗。1)意外伤害发生后,首先去公立和私立医院的急诊室、急救中心、医疗中心、家庭医生诊所和理疗诊所等,医生给出诊断,为患者申请ACC,医生将开出针灸介绍信,病人带介绍信可去针灸治疗。2)意外损伤发生后,也许病人去做其他治疗或检查,如果病人想做针灸治疗,可直接带ACC给病人的信去针灸治疗。3)如果患者受伤后先到理疗诊所做理疗治疗或脊椎诊所做调整脊椎,患者也可同时到针灸诊所做针灸治疗,这样ACC是允许的。4)如果针灸一个疗程后,需要继续针灸治疗,针灸医生可直接为病人再申请最多6次的针灸治疗。5)ACC患者做针灸治疗不是无限制的,每种疾病ACC都限制了针灸治疗次数,如颈部扭伤治疗次数是11次,腰部扭伤是16次等,在ACC限定的针灸治疗次数内患者的病情还没完全恢复,针灸医生还可以给患者再申请最多6次的针灸治疗,但不是无限制的。

最后我想让大家知道,对中医来说,ACC报销只包括针灸方面的相关治疗,这也是中国针灸感到骄傲的地方,因此患者想用中医的其他治疗方法来治疗你的损伤,如中药等,那就要你自己付钱了,ACC是不负责这部分费用。 (阅读更多文章 www.drwin.co.nz 黄博士诊所09 5296185)
